Abstract : Three hundred and four Egyptian cases (608 mandibular second premolars) were screened from the out -patient clinic of Future University in Egypt Dental Hospital. Morphological features of the crown, number and position of cusps as well as the sex of the patient were recorded clinically. The females showed statistically significant higher prevalence of bilateral lower second premolar three cusp type (3 C.T) than males and the vice versa was seen for the males. On the other hand there was no sex predilection in the prevalence of the mixed forms. In both sexes the bilateral 2 C.T variant of lower second premolar was statistically significant higher than the bilateral 3 C.T. However the bilateral variants are statistically significant higher than the mixed ones. In both sexes the 3 C.T variant of lower second premolar in the right side was statistically significant higher than that of the left side.
